Family Support Model (FSM)

In Warrington, we recognise that children and their families have differing levels of need and that services, from preventative through to child protection, should be developed to meet those levels.
Stakeholders from across children’s services have collaborated to agree a model for integrated, multi-disciplinary working to support children and young people in achieving the Every Child Matters five outcomes. This model focuses on the early identification of children and families with additional support needs.
The agreed model aims to promote a shared responsibility between all services working with children and young people, creating a common language and clear procedures for integrated working across all services.
