Service allocation meetings (SAM)

The Warrington CAF and Development Team are responsible for holding and administering service allocation meetings (SAMs).

 

SAMs are held on a regular basis and serve to offer advice and guidance to professionals using common assessment forms and the Warrington family support model.

 

A range of professionals from the following services will attend each SAM:

  • Health
  • Children's Centres
  • Youth Services 
  • Social Care
  • Inclusion Services
  • Warrington Attendance Team
  • Intensive Family Support 
  • Others to be consulted with as needed

 

If you would like to discuss a child, young person or family at a SAM, you must get their signed consent.  Consent should be recorded on a CAF assessment and will only be accepted on a FSM01 referral form in exceptional circumstances.  Copies of CAF forms and referral forms are available in the practitioners toolkit.
