The role

Based at Warrington & Halton Hospital Discharge Team, Kendrick Wing, Warrington Hospital, Lovely Lane, Warrington

The hospital transfer of care team are looking for an enthusiastic, motivated and experienced qualified social worker for a fixed term 1 year contract to cover maternity leave.

The role is dynamic, varied and challenging. The post holder will work as a senior member of the team and

· Hold a small caseload

· Provide management supervision and support to other members of the team

· Provide managerial overview on a day to day basis.

This is an exciting time to join the team, with the introduction of discharge to assess. As part of the management team, you will be able to offer your experience, advice, knowledge base etc to influence the way the team develops this new way of working to ensure that once an illness requiring hospital care is over, it reduces unnecessary time in hospital for people for whom a community or other setting is most appropriate.

Warrington is situated between Liverpool and Manchester with good road and rail connections, providing easy access to all other areas of the country. £107m is being spent on updating the town, providing it with new shops and market hall, a cinema complex, restaurants and council offices. Our team is based close to the town centre at Warrington & Halton Hospital, on a bus route and a short walk from the station.
