Under 65 Service
As a Senior Practitioner working within this service area you will work with adults predominately aged 18 to 65 who have learning disabilities, mental health conditions and/or physical disabilities in the community. We also work with prison offenders, drugs/alcohol dependency. Workers on the team have the opportunity to develop their skills and knowledge around a wide range of issues including Care Act Assessments, Court Of Protection, and Safeguarding enquiries, Continuing Health Care, Best Interest Decisions and Mental Capacity Assessments.
The work involves completing assessments, support plans and reviewing packages of care under The Care Act using a Strengths Based Approach. There is sometimes a requirement to work out of area and/or in the court arena. There is a great emphasis on Integration and we have good links with mental health services and learning disability nursing. We also work closely with the Wellbeing Service.
Over 65 Service
The Over 65 service carry out a care management function working primarily with people over the age of 65 years with a range of complex needs, many with Dementia. Workers on the team have the opportunity to develop their skills and knowledge around a wide range of issues including Care Act Assessments, Court Of Protection, Safeguarding enquiries, Continuing Health Care, Best Interest Decisions and Mental Capacity Assessments.
The service is working in partnership across the town to shape a health and social care system to be proud of, one that’s fit for the future and built to last.

As an experienced social worker you will adopt a strengths based approach to the delivery of high quality care and support. Our social workers make a huge difference day in, day out, and our organisation is playing a leading role in efforts to deliver the truly joined up health and social care services the people of Warrington need and deserve.
Both the Under 65 and Over 65 services cover 4 geographical areas in Warrington, South, West, East and Central. Each team is supported by a Team Manager, a Senior Practitioner, Experienced Social Workers, Social Workers and Community Care Workers. The teams also have access to admin support.
As a Senior Practitioner you will support the Team Manager and staff within the teams to deliver a safe, effective service for adult service users, carers and families with a focus on the following key areas:-
• Allocation of cases
• Mentoring role
• Legal literacy
• Complaint investigation
• HR issues
• Holding complex cases
• Deputising for the Team Manager
As a valued team member you will be fully supported with your individual case management caseload. You will work within the legislation of the Care Act, Mental Capacity Act and the Mental Health Act in order to deliver personalised assessment, support planning and reviews following the strengths based approach, networking with partners and ensuring holistic outcomes.
