An exciting opportunity has arisen for Grade 7/8 social workers to apply to Warrington's Over 65 Service

We are working in partnership across the town to shape a health and social care system to be proud of, one that’s fit for the future and built to last. 

We require the very best social work professionals on board with a strengths based approach to the delivery of high quality care and support. Our social workers make a huge difference day in, day out, and our organisation is playing a leading role in efforts to deliver the truly joined up health and social care services the people of Warrington need and deserve.

We currently have a number of vacancies within the Over 65 Service including full time posts of 37 hours and the possibility part time posts with hours to be negotiated.  The service covers 4 geographical areas in Warrington; South, West, East and Central. Each team is supported by a Team Manager, a Senior Practitioner, Experienced Social Workers, Social Workers (including ASYE) and Community Care Workers. The teams also have access to admin support.

The Over 65 service carry out a care management function working primarily with people over the age of 65 years with a range of complex needs including Dementia. Workers on the team have the opportunity to develop their skills and knowledge around a wide range of issues including Care Act Assessments, Court Of Protection, Safeguarding enquiries, Continuing Health Care, Best Interest Decisions and Mental Capacity Assessments.  You will work within the legislation of the Care Act, Mental Capacity Act and the Mental Health Act in order to deliver personalised assessment, support planning and reviews following the strengths based approach, networking with partners and ensuring holistic outcomes.  As a valued member you will be fully supported with your individual case management caseload.
