When you turned 16, your social worker will have discussed your Pathway Plan with you.

A Pathway Plan will set out the support that you will be given once you have left care. This must be based on a needs assessment and should be prepared before you leave care. Your plan will be reviewed every 6 months.

At 16 you will also be allocated a Personal Advisor.

A Personal Advisor (PA) is someone who acts as a focal point to make sure that you get the right support as you move into adulthood and transition from being in care.

They will remain with you, offering support right through until you turn 21, or 25 if you choose. Your Personal Advisor will then review your Pathway Plan with you during your early adult years.

When reviewing your Pathway Plan, we usually cover certain issues to make sure that we support you fully through the transition process. This includes your health and wellbeing, money, relationships, education, employment, housing, participation in society, advocacy and more.
